Nosocomial bacteraemia (NB) is an important, composite indicator of clinical performance, representing sepsis or failure of aseptic sampling technique. It increases hospital costs, may be a
marker of overcrowding and understaffing, and could have major medicolegal implications. Risk adjusted comparisons of NB thus merit priority. Gray et al showed that risk of NB is related to
illness severity as measured by SNAP (Pediatrics 1995; 95: 225-30). As CRIB (Lancet 1993;342:193-8) is simpler it may be a more appropriate measure of risk and illness severity in large
multicenter studies. We assessed the relationship between CRIB and NB, defined as one or more episodes of suspected sepsis with positive blood culture arising > 48 hours after birth. On
retrospective analysis, 143 (99%) of 145 infants of birth weight<1500g or gestation <31 were admitted to Ninewells Hospital NICU in 1992-94. 38 infants (26%) had NB and 72% of the
isolates were coagulase negative staphylococci (C-NS). On Cox regression, a one point increase in CRIB was associated with a relative risk (RR) of NB of 1.12 (95% Confidence Interval (CI)
1.01, 1.24), an extra week in gestation at birth was associated with a RR of 0.80 (95% CI 0.67, 0.96) and no other variable entered the model. Infants with higher CRIB scores thus developed
NB more rapidly(Figure). CRIB and gestation may be important explanatory variables in multicenter comparisons. Separate models may be needed for C-NS and for other isolates. _Supported by
